Health Outreach and Enabling Services

Health Outreach and Enabling Services represent a broad spectrum of activities that facilitate access to health services, including case management, transportation, interpretation, and helping patients navigate the health care system. 

HOP offers a range of training and consultation services addressing Health Outreach and Enabling Services, including:

  • Coordinating Health Outreach Programs Training – hands-on learning in outreach program planning and evaluation, supervising program staff, strengthening data collection mechanisms, and time management.
  • Basic Outreach Skills Training – interactive information for outreach staff on how to plan and implement culturally-appropriate outreach programs and activities that best meet the needs of their communities.
  • Outreach for Benefits Enrollment Training – outreach skills and techniques for enrolling families into Medicaid, CHIP, and other public benefits programs. 
  • Transportation Models – consultation services or training modules on strategies for developing an appropriate transportation model that addresses the local context and population needs.
  • Outreach Program Assessment Consultation – assessment of an outreach program’s structure, services provided, staff, client demographic characteristics, and community resources to identify areas of strength and need.
  • Other Customized Outreach Services – HOP can customize its training and consultation services to help meet the specific needs of the populations you serve.
