Health Education and Promotion

Health Education and Promotion are key components of a patient-centered health care model, enabling individuals to become active partners in their own health care.

HOP offers a range of training and consultation services addressing Health Education and Promotion, including:

  • Coordinating Health Outreach Programs Training – hands-on learning in outreach program planning and evaluation, supervising program staff, strengthening data collection mechanisms, and time management.
  • Outreach Program Assessment Consultation – assessment of an outreach program’s structure, services provided, staff, client demographic characteristics, and community resources to identify areas of strength and need.
  • COCHE™ (Curriculum for Outreach Centered Health Education) Training – interactive instruction for outreach staff on how to conduct effective health education which empowers patients to play an active role in their health.
  • COCHEcito Training – a comprehensive outreach services model and health education curriculum to reach children ages 0-5 years.
  • Health Education Consultation – specific strategies for conducting meaningful, culturally-appropriate health education.
  • Consultation on Outreach and/or Health Education Materials (including Review and Feedback) – information on developing culturally-appropriate outreach and health education materials; review and feedback on existing materials.
