Needs Assessment and Evaluation Data increase access to care by allowing organizations to design responsive programs, evaluate and improve upon activities, and document the effectiveness of outreach. 

HOP offers a range of training and consultation services addressing Needs Assessment and Evaluation Data, including:

  • Coordinating Health Outreach Programs Training – hands-on learning in outreach program planning and evaluation, supervising program staff, strengthening data collection mechanisms, and time management.
  • Conducting a Community Needs Assessment Training – information on defining the parameters of community needs assessment efforts, selecting appropriate data collection methods, addressing data analysis considerations, and utilizing information obtained to provide services to underserved communities.
  • Outcome Evaluation Training – step-by-step process of outcome evaluation to organize the components of the outreach program to effectively measure the results of outreach efforts.
  • Needs Assessment Consultation – facilitation of needs assessment planning and design process, and feedback on data collection tools and assessment activities. 
  • Data Collection Assistance – consultation and training modules on appropriate data collection techniques to use with underserved populations. Advice, review and comment on data collection tools.
  • Program Outcome Measures Assistance – consultation and training modules on strategies for evaluating the outcomes of outreach program and individual outreach activities.
