Peer-to-Peer Events

HOP’s Peer-To-Peer Events are designed to give health organizations and public health professionals a real-life glimpse into a variety of health topics currently impacting underserved populations throughout the United States, such as farmworkers, low-income populations, immigrants, homeless, and others. During these events, representatives from health and outreach programs will impart their knowledge and share their firsthand experiences about the topics at hand. HOP believes that health outreach workers, promotores, and program administrators will gain invaluable information and insight by attending these events.

Each HOP Peer-To-Peer Event hosted via Webex (a web-based platform for sharing resources, presentations, and connecting through a teleconferencing system) will consist of a facilitated discussion between panelists and the event host.  By attending, you’ll have the opportunity to pose real-time questions relevant to the event topic.  If you have an interest in improving outreach services for underserved populations, please join HOP to learn how the featured topics impact your work.

The Role of Outreach Programs in Improving Health Outcomes

Farmworkers have a disproportionately higher disease burden compared to the general population, yet they are significantly less likely to use preventive health services. Health outreach programs play a critical role in extending and improving the health care provided to farmworkers. Fifty percent (50%) of respondents to Health Outreach Partners 2010 national needs assessment said that health education is one of their most common outreach activities, and nearly 40% reported basic health screenings as a key outreach activity.  In this event, we will explore the unique strengths of health center outreach programs in improving patient care and health outcomes. Administrators, outreach staff, and clinicians alike are invited to examine specific outreach practices that improve health outcomes while also discussing approaches to measuring them.

Poison Prevention – Strategies and Techniques to Enhance Your Skills for Reaching Low-Income Communities

Did you know there are more than 4 million poisonings annually in the U.S. and over half of these are with children under the age of five?  Did you know that people living in impoverished neighborhoods have a disproportionate risk of being poisoned, as compared to the general population?  HOP’s Peer-to-Peer Event on poison prevention will explore some of the reasons why these populations are more susceptible to poisonings, plus what outreach staff can do to arm themselves with the most updated and relevant information about this pressing health topic.

 The Role of Outreach in the Patient-Centered Medical Home

Because of escalating medical care costs and national health care reform, community health centers are receiving well-deserved praise and attention for their cost-effective and high-caliber health services. Community health centers are particularly well suited to adopt the Patient Centered Medical Home (PCMH) model, a health delivery system which incorporates finely coordinated medical care that responds to the needs of the “whole person.” Patients from underserved and vulnerable populations can especially benefit from the personalized and detailed care found within the PCMH model. Community health workers, outreach workers, promotores de salud, and health educators are uniquely positioned to identify and respond to the various medical and social service needs of different populations in their area, which fuels a well-coordinated and successful PCMH.  Panelists will examine how outreach programs are integral to the PCMH team.

Exploring How Knowledge of Legal Rights Impacts Health Care Access among Immigrant Communities

Panelists will examine techniques and special considerations for delivering legal rights education and information to immigrant communities.  The connection between legal rights education initiatives and health care access in a climate where public discourse often associates immigrants with overburdened public health insurance programs will also be discussed.
