Welcome to Outreach Connection, an information portal for outreach staff. HOP created this portal for community organizations and advocacy groups that seek to build and sustain programs that improve people's health and well-being. Information included in Outreach Connection will address health outreach and enabling services, program planning and development, needs assessment and evaluation data, health education and promotion, community collaboration, and cultural competency.
