Health Outreach Partners (HOP) is a national non-profit health organization providing training, consultation, and information services to community-based organizations striving to improve the quality of life of hard-to-reach populations.
Since 1970, HOP has been at the forefront of elevating the importance of outreach, recognizing the critical role it plays in facilitating case management, primary care, health prevention and social services to underserved communities. By professionalizing outreach services, HOP has established itself as the leader in enhancing health organizations' ability to successfully reach farmworkers and other vulnerable populations and offer critically-needed services.
Priority Areas
HOP focuses on six priority
areas that aim to increase access to care, quality of health services,
and organizational sustainability.
Health Outreach and Enabling Services represent a broad spectrum
of activities that facilitate access to health services, including case
management, transportation, interpretation, and helping patients navigate
the health care system.
Program Planning and Development are fundamental processes used
to strategically set goals, maximize resources, and ultimately make
organizations more sustainable.
Needs Assessment and Evaluation Data increase access to care by
allowing organizations to design responsive programs, evaluate and improve
upon activities, and document the effectiveness of outreach.
Health Education and Promotion are key components of a patient-centered
health care model, enabling individuals to become active partners in
their own health care.
Community Collaboration and Coalition Building are critical strategies
that ensure a comprehensive, complementary, and sustainable system of
care in the community.
Cultural Competency values the unique characteristics of diverse
populations and ensures quality health care that is both respectful
and responsive.
